Complete an Employee Separation Notification Form

Overview

This article details the steps to complete an Employee Separation Notification Form (105-021).

Steps

  1. Go to https://eforms.nvcc.edu/FormsLibrary/Dashboard.aspx

  2. Enter your NVCC username (your local username, not your NVCC email address) and password.

  3. Click the Log in button.

  1. The Forms Library dashboard displays. Click eForms in the left panel or under Forms Library to view the forms.

  1. Click Employee Separation Notification Form (105-021) in left panel or under EForms.

  1. The form displays. Complete the information in the Employee section.

Note: If your Administrative Unit or supervisor is incorrect, please contact DataOps@nvcc.edu for assistance before proceeding with this form.

Note: In the event an employee chooses to submit a letter of or email resignation to their supervisor, the supervisor will respond to the employee acknowledging receipt and acceptance of the resignation. The supervisor will then submit the 105-021 based on receipt of this notification. A copy of each resignation email or letter must be sent to Human Resources and should include the employee’s name, position, location, and the effective date of resignation. Otherwise, employees can electronically submit the Employee Separation Notification Form (105-021). Please do not use this form when an employee transfers to another position in the College.

  1. Select your reason(s) for leaving (you can select more than one option) and enter your reason description in the Comments.

  1. Check the box and complete the Forwarding Address fields if applicable.

  1. Carefully read the Acknowledgement and then click the Submit button.
