This article details the steps to complete an IT Accounts Request Form (105-045).
Go to https://eforms.nvcc.edu/FormsLibrary/Dashboard.aspx
Enter your NVCC username (your local username, not your NVCC email address) and password.
Click the Log in button.
The Forms Library dashboard displays. Select the eForms pane to view the forms.
Click the IT Accounts Request link.
The form displays. Select the Reason for the request.
In the Employee section, enter your email. The EMPLID, Last Name, First Name, Title, and Supervisor fields will automatically populate.
If applicable, select the application in the drop-down and click the Add to Cart button. If the access is not in the drop-down, please select Unlisted Application and enter the additional information at the bottom of the form.
Carefully review your request information and then click the Next button.
The Success confirmation dialog displays. The form will be sent to your supervisor for approval. Click the Cancel button to exit or the Submit New Request button to submit another request.
