Overview
This article details the steps to complete an Adjunct Faculty IT Account Request (105-045) for a new adjunct. There are two ways to submit a request for an IT account for a new adjunct: the Forms Library dashboard or IT Accounts Request Form (105-045).
Steps

Go to https://eforms.nvcc.edu/FormsLibrary/Dashboard.aspx

Enter your NVCC username (your local username, not your NVCC email address) and password.

Click the Log in button.

The Forms Library dashboard displays. Select the Adjunct Hires pane.

The form displays. In the Adjunct Information section, enter your EMPLID, First Name, Last Name, Supervisor’s Email. Select your Administrative Unit and Division.

If the request is for a Dual Enrollment Adjunct, select the Dual Enrollment Adjunct checkbox.

Click the Add to Cart button.

Carefully review your request information and then click the Submit button.

The Success confirmation dialog displays. The form will be sent to your supervisor for approval. Click the Cancel button to exit or the Submit New Request button to submit another request.
Alternately, you can submit a request for an Adjunct Faculty IT Account Request by opening an IT Accounts Request Form (105-045).

Select Adjunct Request at the top-right of the IT Accounts Request Form (105-045).

The form displays. In the Adjunct Information section, enter your EMPLID, First Name, Last Name, Supervisor’s Email. Select your Administrative Unit and Division.

If the request is for a Dual Enrollment Adjunct, select the Dual Enrollment Adjunct checkbox.

Click the Add to Cart button.

Carefully review your request information and then click the Submit button.
