View the Forms

Overview

This article details the steps to view the forms available in the Service Catalog in the NOVA Services Portal.

Steps

  1. Go to the NOVA Services Portal: https://services.nvcc.edu/

  2. Select Sign In in the top right corner.

  1. Enter your myNOVA (VCCS) username and then click the GO button.

  1. Enter your myNOVA (VCCS) password and then click the GO button.

  1. Use Multi-factor Authentication (MFA) to login. Please refer to MFA for myNOVA (VCCS) for the detailed steps.

  2. Once authenticated, click the Forms button or select Forms in the navigation bar.

  1. The Forms page displays, listing available forms in the Service Catalog. Select a form link to view the IT service connected to the form.

  1. Click the form button on the right to view the form.

  1. Complete the form fields and then click the Submit button to submit the form. You will receive email notification(s) of the progress of the request, from submission to completion.
